Game Change Request is the first step in requesting to change a CJSL Scheduled Game. The form below must be completed and then approved by the CJSL Games Chairman. Approval does NOT mean the Game has been changed. The team/club must then ask the other team to change the game. After all of the details (new time; date; location;)have been worked out the team requesting the Game Change must fill in and complete the Game Change Form. The Game Change Request and Game Change Form are required for any game change - including - Changing a Game because of State Cup conflict; Changing Game Date; Changing Game Time; Changing Game Location.

1. Game Change Fee is $50.00.
2. The Game Change Request Form must be submitted at LEAST 14 Days (2 Weeks) prior to the     Scheduled Game.
3. The $50.00 Fee must be in the office before the FINAL Game Change approval or denial will be     sent.
4. Game Change Request Form MUST be APPROVED prior to completing Game Change Form.
5. A Submission Number is generated automatically upon completion of Game Change Request.     The Submission number that is generated is required on Game Change Form.
6. State Cup Game Change Rules: All Game Change fees will be waived if the Game Change     Request and Game Change Form are completed 10 days prior to date of the game. A $25.00     Fee will be ASSESSED if the Game Change Request and Game Change Form are completed     LESS THAN 10 days prior to date of the game.
7. Weather Related Game Change Rules: All Game Change fees will be waived if the Game     Change Request and Game Change Form are completed 14 Days (2 Weeks) prior to date of     the game. A $25.00 Fee will be ASSESSED if the Game Change Request and Game Change     Form are completed LESS THAN 14 Days (2 Weeks) prior to date of the game.
8. If you do not send in the fee the game will not be changed.
9. YOU WILL BE NOTIFIED BY EMAIL IF YOUR GAME CHANGE REQUEST HAS BEEN
    APPROVED.
